Zusammenfassung
Anorchie bedeutet Nicht-Anlage oder Verlust eines oder beider Hoden bei genetisch männlichen Individuen. Demnach wird zwischen angeborener oder erworbener Anorchie unterschieden. Angeborene Anorchie ist ein seltenes Ereignis mit einer Prävalenz von 1:20.000 Neugeborenen. Zur Diagnose der beidseitigen Anorchie dienen der hCG-Test und die Bestimmung des Anti-Müller-Hormon (AMH). Erworbene Anorchie kann Folge von Torsionen, Traumen, Infektionen, missglückten Operationen oder Kastration sein. Eine Testosteron-Substitution ist das Mittel der Wahl, um eine Androgenisierung einzuleiten oder zu erhalten. Nach Kastration aus medizinischer Indikation (z. B. Prostatakarzinom) oder aus sozio-kulturellen Gründen würde eine Testosteron-Substitution den Zweck der Kastration verfehlen.
Bei einer Polyorchie werden mehr als zwei Hoden entdeckt, am häufigsten sind Triorchien, aber auch das Vorkommen von fünf Hoden wurde beschrieben. Polyorchien bleiben meist symptomlos und werden zufällig entdeckt z. B. durch Sonografie oder MRT, die auch einen Verdacht bestätigen können.
