Operative Therapie der Colitis ulcerosa und Pouchitis

Zusammenfassung

10–20 % aller an Colitis-ulcerosa-Erkrankten müssen sich einer Proktokolektomie unterziehen. Bei korrekter Indikationsstellung profitieren die Patienten am meisten von einer restaurativen Koloproktomukosektomie mit ileo-J-Pouch-analer Anastomose. Dieses Verfahren beschert 90 % aller Operierten eine exzellente Lebensqualität. Allerdings handelt es sich um ein komplexes und komplikationsreiches Verfahren, was ein entsprechendes Management des behandelnden Chirurgen erfordert. Daher sollte diese Operation hauptsächlich in dafür spezialisierten Zentren erfolgen. Eine häufige Komplikation ist die Pouchitis, die meisten gut zu behandeln ist. Ein geringer Prozentsatz der Patienten erleidet eine chronische Pouchitis, die nach wie vor sehr schwierig zu beherrschen ist und oft in einer Pouchexstirpation mit endständigem Ileostoma endet.
