Zusammenfassung
Eine chirurgische Therapie ist heute bei erworbenen und infektiösen Erkrankungen der Lunge und der Pleura nur noch selten notwendig. Infektiöse Erkrankungen werden mit hochpotenten Antiinfektiva behandelt und die Möglichkeiten der interventionellen Therapie (z. B. Fibrinolyse bei Pleuraempyem oder Drainageeinlage beim Lungenabszess) sind vielfältiger geworden und werden aufgrund ihres minimal-invasiven Charakters zu Recht konsequent genutzt. Auch im Bereich der chronischen Lungenerkrankungen (z. B. Bronchiektasen bei Mukoviszidose) ist heute der Chirurg nicht mehr so häufig gefordert, wie es noch vor 20 Jahren der Fall war. Allerdings haben sich auch neue Tätigkeitsfelder ergeben. Beispielsweise ist die Notwendigkeit der Lungenbiopsie heute aufgrund der immer differenzierteren Klassifikation der interstitiellen Lungenerkrankungen deutlich häufiger geworden. Zudem gibt es in allen Bereichen immer wieder Patienten, bei denen die medikamentöse oder interventionelle Therapie nicht zum Ziel führt. Daher ist in der pädiatrischen Thoraxchirurgie eine enge interdisziplinäre Zusammenarbeit mit den pädiatrischen Pulmonologen zielführend.
