Zusammenfassung

Die Dynamik der Ausbreitung bakterieller Resistenzen lässt befürchten, dass in absehbarer Zeit die Wirksamkeit und die Anzahl der zur Verfügung stehenden Antibiotika deutlich eingeschränkt sein werden. Insbesondere bei multiresistenten gramnegativen Bakterien (MRGN) und Vancomycin-resistenten Enterokokken (VRE) sind derzeit unsere therapeutischen Möglichkeiten begrenzt. Die Notwendigkeit einer schnellen Therapieeinleitung zwingt jedoch bei kritisch Kranken zu einer initial breiten antibiotischen Therapie, da ein zuverlässiges mikrobiologisches Ergebnis, inkl. Resistogramm, frühestens nach 36–48 h vorliegt. In dieser Situation empfehlen die Leitlinien der Deutschen Sepsis-Gesellschaft (DSG) und der Deutschen Interdisziplinären Vereinigung für Intensiv- und Notfallmedizin (DIVI), ein breit wirksames Antibiotikum einzusetzen.
