Zusammenfassung
Mastzellen stammen von CD34+ hämatopoetischen Knochenmarkszellen ab, exprimieren den Tyrosinkinase-Rezeptor KIT (CD 117) und den hochaffinen IgE-Rezeptor Rezeptor FcεRI und sind prall mit Granula gefüllt, die reichlich Entzündungsfaktoren enthalten. Im Gewebe sind die konstitutiv vorhandenen Bindegewebs- und Schleimhautmastzellen unterscheidbar. Die Mastzelldichte im Gewebe kann durch Neoplasien und entzündliche oder aktivierende Prozesse erhöht sein. Bei systemischen Mastzellvermehrungen spielt häufig eine aktivierende KIT-Mutation (meist D816V) eine Rolle. Einen definierten Grenzwert der physiologischen Mastzelldichte gibt es nicht. Der histologische Nachweis von Mastzellen beruht auf ihren speziellen Färbeeigenschaften: metachromatische Granula in der Toluidinblau- und Giemsafärbung, positive Reaktion in der ASD-Chloracetatesterasefärbung, Immunfärbungen für Tryptase, Chymase, CD 117 und IgE-Rezeptor. Elektronenmikroskopisch können die Bindegewebs- und Schleimhautmastzellen differenziert werden. Die häufigsten Formen der Mastozytosen betreffen die Haut, bei Kindern vor allem als isolierte Mastozytome, bei Erwachsenen eher als Urticaria pigmentosa oder Teleangiectasia macularis eruptiva perstans. Diffuse kutane Mastozytosen, systemische Formen oder maligne Mastozytosen sind dagegen sehr viel seltener. Vor einer Biopsie sollten mastzelldegranulierende Einwirkungen wie mechanische Reizung oder direkte Infiltration mit Lokalanaesthetika, die die Detektion der Mastzellen erschweren können, vermieden werden. In der Biopsie zeigt sich eine erhöhte Mastzelldichte. Je nach Art der Mastozytose können die Mastzellen dabei unterschiedlich geformt und im Gewebe verteilt sein und es können weitere Veränderungen wie pseudoepitheliomatöse Hyperplasie, Akanthose, Hyperpigmentierung, Angieektasien, Ödem, Blasenbildung, Fibrose, Infiltrate aus Melanophagen, eosinophilen und neutrophilen Granulozyten oder Lymphozyten beobachtet werden. Der histologische Befund sollte in Zusammenschau mit der Klinik interpretiert werden.
