Zusammenfassung
Hauptziel der Geburtshilfe ist die Reduktion der maternalen und perinatalen Mortalität und Morbidität. Ein perineales Trauma kann sich spontan oder sekundär trotz Episiotomien ereignen, deren protektiver Wert kontrovers beurteilt wird. Höhergradige Dammrisse erfordern besondere Aufmerksamkeit, weil nicht erkannte oder nicht adäquat versorgte Dammrisse, die den Kontinenzmechanismus einbeziehen, erhebliche Auswirkungen haben.
Vaginaloperative Geburten zeigen schlechtere Resultate hinsichtlich Beckenbodentraumata. Die elektive Sectio hat einen gewissen präventiven Wert, um Urininkontinenz unmittelbar postpartal zu reduzieren, die Langzeitresultate sind allerdings noch unklar. Wassergeburten haben weder einen positiven noch einen negativen Einfluss.
Eine definitive Beurteilung nach postpartalen Beckenbodenproblemen – auch bzgl. eventueller operativer Sanierung – sollte erst nach Abschluss der Stillzeit, wenn die Frau wieder regelmäßige Zyklen hat und hormonell kein Hypoöstrogenismus vorliegt.
