Salbutamol (Aerosol und Zäpfchen) bei Kindern mit Asthma bronchiale

In 10 children with asthma bronchiale aged \(4{\raise0.7ex\hbox{${10}$} \!\mathord{\left/ {\vphantom {{10} {12}}}\right.\kern-\nulldelimiterspace} \!\lower0.7ex\hbox{${12}$}} - 14{\raise0.7ex\hbox{$3$} \!\mathord{\left/ {\vphantom {3 {12}}}\right.\kern-\nulldelimiterspace} \!\lower0.7ex\hbox{${12}$}}\) years, Salbutamol was given rectally and by pressurized aerosol. After 1 h plasma concentration of free salbutamol and lung function were measured. Plasma concentrations of free salbutamol ranged between 1.2–12.3 ng/ml, when 2 mg of salbutamol were given rectally and between < 0.5–1.6 ng/ml, when a dose of 0.2 mg salbutamol was inhaled. Airway resistance decreased significantly after both forms of treatment but there was no statistical difference. Plasma concentrations of free salbutamol were rather low following an aerosol dose of 0.2 mg salbutamol, suggesting a topical action of salbutamol. When taken rectally improvement in lung function seemed to be related to plasma levels of free salbutamol.

10 Kinder mit Asthma bronchiale im Alter von \(4{\raise0.7ex\hbox{${10}$} \!\mathord{\left/ {\vphantom {{10} {12}}}\right.\kern-\nulldelimiterspace} \!\lower0.7ex\hbox{${12}$}} - 14{\raise0.7ex\hbox{$3$} \!\mathord{\left/ {\vphantom {3 {12}}}\right.\kern-\nulldelimiterspace} \!\lower0.7ex\hbox{${12}$}}\) Jahren erhielten ein Salbutamol-Zäpfchen zu 2 mg. Eine Stunde nach Gabe schwankten die Plasmakonzentrationen an freiem Salbutamol zwischen 1,2–12,3 ng/ml. Inhalierten die Kinder 0,2 mg Salbutamol aus einem Dosieraerosol, so lagen die Werte für freies Salbutamol im Plasma eine Stunde später in einem Bereich von < 0,5–1,6 ng/ml. Der bronchiale Strömungswiderstand fiel sowohl nach Applikation des Zäpfchens als auch nach Inhalation des Aerosols signifikant ab. Zwischen den beiden Arzneiformen ließ sich in der bronchialen Wirkung kein statistisch signifikanter Unterschied errechnen. Ein Vergleich der Plasmaspiegel an freiem Salbutamol zeigt bedeutend niedrigere Konzentrationen nach Aerosolinhalation im Vergleich zur Zäpfchengabe. Es wird vermutet, daß das Aerosol lokal angreift, die broncho-lytische Wirkung des Zäpfchens scheint dagegen an die Höhe des Blutplasmaspiegels gebunden zu sein.
