Möglichkeiten und eigene Erfahrungen beim endoprothetischen Kniegelenkersatz

Summary

The main problem with any endoprosthetic joint is permanent fixation in the bone. The biomechanical problems in fixation of the prosthesis in the preponderantly spongy bone at the knee joint are different from those encountered at the hip. Everything is in favor of the total endoprosthesis and nothing of a partial prosthesis. Guepar total prostheses have been inserted in 32 patients. In most of these cases the indication was severe painful arthrosis deformans, more rarely it was chronic rheumatism, and in one case of the fragmented fracture tibia head. No postoperative disturbances or mechanical defects have so far been observed.

Zusammenfassung

Hauptproblem jeder Gelenkendoprothese wird die dauerhafte Verankerung im Knochen sein. Die biomechanischen Probleme sind mit der Verankerung der Prothese im überwiegend spongiösen Knochen am Kniegelenk anders als an der Hüfte. Alles spricht für die Totalendoprothese und gegen die Teilprothese. 32 mal wurde eine Guépar-Totalprothese eingesetzt. Die Indikation war hauptsächlich die schwere, schmerzhafte Arthrosis deformans, seltener der chronische Rheumatismus und einmal eine Tibiakopf-Trümmerfraktur. Postoperative Störungen oder mechanische Defekte wurden bisher nicht beobachtet.
