Hepatitis B Virus Infection and Primary Glomerular Disease

Summary

Since the first report of a possible pathogenetic association between persistent antigenemia and membranous Australian glomerulonephritis (GN) in 1951, the implications of hepatits B virus (HBV) on the pathogenesis of primary glomerular disease, particularly in the Asian region, have been extensively discussed.

Now the association of the HBV infection with membranous nephropathy (MN) in children is well established. There is a close relationship between the frequency of the HBsAg carrying state in MN and the underlying prevalence of HBsAg carriage in the general population. The HBV-MN in children is also unique in its clinical presentation, serology, and immunopathology, with HBeAg as the dominant type of HBV antigen deposited in glomeruli.

However, the investigation of HBV-GN relied mainly upon the demonstration of HBV specific antigens in the glomeruli which was subject to many technical errors and discrepancies. Opinions about the pathogenetic association between HBV and other types of primary GN (PGN) are still controversial.
