Behandlung älterer Patienten mit Clozapin

Zusammenfassung

Clozapin ist wegen seiner besonderen pharmakologischen Eigenschaften für Patienten, die auf konventionelle Neuroleptika nicht respondieren oder deren Nebenwirkungen nicht tolerieren, eine sinnvolle Alternative. Dies gilt auch für gerontopsychiatrische Patienten. Therapierefraktäre paranoide Syndrome und psychotische Entgleisungen bei M. Parkinson sowie Versuche der Tre-morreduktion stellen Indikationen dar, in denen Clozapin auch bei alten Patienten eingesetzt wird. Bei Durchsicht der Literatur zeigt sich jedoch ein großer Mangel an gesicherten empirischen Daten zur Wirksamkeit und Sicherheit von Clozapin bei Patienten dieser Altersgruppe. Aufgrund der Datenlage kann aber davon ausgegangen werden, daß man mit Clozapin auch bei alten Menschen in unterschiedlichen Indikationsbereichen gute Behandlungserfolge erzielen kann. Die Behandlung alter Patienten mit Clozapin erfordert jedoch besondere Umsicht und Kenntnis der pharmakodynamischen und pharmakokinetischen Besonderheiten im Alter.
