Zusammenfassung

Verschlechterung eines bereits bestehenden Kopfschmerzes wird gemäß der vorbestehenden Form verschlüsselt. Patienten, die in enger zeitlicher Beziehung zu einem Kopftrauma eine neue Kopfschmerzform entwickeln (einschließlich Migräne, Kopfschmerz vom Spannungstyp und Clusterkopfschmerz), werden in Gruppe 5 verschlüsselt. Der Kopfschmerztyp kann dann an der 4. Stelle des Codes spezifiziert werden (s. oben). Das posttraumatische intrakranielle Hämatom wird zur Gruppe 6, der posttraumatische Hydrozephalus zur Gruppe 7 gezählt. Eine kausale Beziehung zwischen Kopftrauma und Kopfschmerz ist häufig anzunehmen bei 5.1.1, 5.1.2 und 5.2.1 und weniger häufig bei 5.2.2. Es bleibt jedoch eine Gruppe von Patienten übrig, insbesondere bei Akzelerations- bzw. Deakzelerationsverletzungen, für die nicht die Kriterien von 5.2.1 A zutreffen, die nach dem Trauma aber einen Knick in der Arbeitsleistung und/oder den sozialen Funktionen oder auch Persönlichkeitsveränderungen zeigen. Dies kann auf eine mögliche kausale Verknüpfung von Kopfschmerz und Trauma hindeuten. Chronische posttraumatische Kopfschmerzen (5.2.1 und 5.2.2) sind oft Teil eines posttraumatischen Syndroms. Die komplexe Wechselbeziehung zwischen organischen und psychosozialen Faktoren ist schwer zu erfassen.
