Zusammenfassung
Die sog. peripher wirksamen Analgetika haben nur ein begrenztes Einsatzgebiet bei der Behandlung von starken Schmerzzuständen. Diese Einschränkung ergibt sich aus ihrem relativ schwachen analgetischen Maximaleffekt, der etwa dem von 10 mg Morphin entspricht. Somit sind diese Medikamente i. allg. nicht geeignet für die frühe postoperative Phase nach größeren abdominalchirurgischen Eingriffen, nach den meisten größeren orthopädischen, perianalen und genitalen Operationen sowie nach vielen zahn-, gesichts- und mundchirurgischen Eingriffen. Selbst Operationen, die eine relativ geringe Gewebstraumatisierung zur Folge haben, wie z. B. Herniotomien oder Varizenentfernung, bewirken häufig zunächst Schmerzen, die den Einsatz von Opiaten erforderlich machen.
