Summary

From March 1988 to January 1990, 125 bronchoscopies were performed on 75 patients on the postoperative intensive care unit; 49 patients were intubated and respirated. The main indications for postoperative bronchoscopy (n = 98) were secretion and the atelectasis formation. In 85% of the cases, specific bronchial lavage, achieved an improvement that was radiologically visible and could be confirmed by bloodgas analysis. Other indications included lung infiltrations (n = 16), where bronchoscopy was curried out to acquire material for bacteriological tests and determination of resistance. After aspiration (n = 7) the incidence of postaspiratory pneumonia (n = 2) could be reduced due to the early indication for bronchoscopic lavage and removal by suction.

Zusammenfassung

Von März 88 bis Januar 90 führten wir auf der postoperativen Intensivstation 125 Bronchoskopien bei 75 Patienten durch, 49 Patienten waren intubiert und beatmet. Die Hauptindikation zur postoperativen Bronchoskopie (n = 98) war die Sekretretention und Atelektasenbildung. In 85% der Fälle konnte durch die gezielte Bronchiallavage eine radiologisch sichtbare und blutgasanalytisch meßbare Verbesserung erzielt werden. Weitere Indikationen waren Lungeninfiltrate (n = 16) zur gezielten Materialgewinnung zur bakteriologischen Untersuchung und Resistenzbestimmung. Nach Aspiration (n = 7) kann durch die frühzeitig gestellte Indikation zur bronchoskopischen Lavage und Absaugung die Inzidenz der Postaspirationspneumonie (n = 2) gesenkt werden.
