Thoracic Trauma in Patients with Multiple Injuries — Prehospital Management

Summary

In 1030 polytraumatized patients (1978–1990) we found 63.6% with thoracic trauma. The rate of patients with contusion of the lung who arrived of the hospital intubated increased from 44.8% (1979-1983) to 56.2% (1985-1989). The survival rate of these patients increased from 67.9% to 85.4%.

The mortality rate of all patients with thoracic trauma decreased from 20.8% to 14.3%. Early intubation, PEEP ventilation and — if needed — chest tube are important factors that can decrease mortality rate in patients with thoracic trauma.

Zusammenfassung

Unter 1030 polytraumatisierten Patienten (1978–1990) fanden wir 63,6% mit Thoraxtrauma. Der Anteil von Patienten mit begleitender Lungenkontusion, der die Klinik intubiert erreichte stieg von 44,8% (1979–1983) auf 56% (1985–1989). Die Überlebensrate dieser Patienten stieg von 67,9% auf 85,4%. Die Letalität aller Patienten mit begleitendem Thoraxtrauma sank gleichzeitig von 20,8% auf 14,3%.

Frühzeitige Intubation, PEEP-Beatmung und nötigenfalls Thoraxdrainage sind wichtige Faktoren, die die Letalität polytraumatisierter Patienten mit begleitenden Thoraxverletzungen senken können.
