Zusammenfassung
Die diabetische Neuropathie ist eine häufige und oftmals schwerwiegende Komplikation des Diabetes mellitus. Sie betrifft sowohl das periphere als auch das autonome Nervensystem und führt zu unterschiedlich komplexen Syndromen, die dem Patienten, insbesondere mit zunehmender Dauer der Erkrankung, einen hohen Leidensdruck auferlegen. Die zahlenmäßig wichtigste diabetische Polyneuropathie ist die Erkrankung sensorischer, peripherer Nerven mit distaler Ausprägung in den Extremitäten. Sie führt sowohl zu einer Steigerung als auch Abnahme der Empfindungen für Berührung, Temperatur, Schmerz sowie zur Einschränkung der Beweglichkeit. Die Sensibilitätsstörungen beginnen in der Regel zunächst an den Beinen und Füßen, mit fortschreitender Dauer des Diabetes machen sie sich jedoch auch an den Armen und Händen bemerkbar und betreffen myelinisierte und nicht myelinisierte dünne Nervenfasern (Aδ- und CFasern). In der Regel wird der Sensibilitätsverlust dem Patienten zunächst nicht bewußt, er kann jedoch bereits zu einem sehr frühen Zeitpunkt der Erkrankung mit sensitive Untersuchungsmethoden gut reproduzierbar quantifiziert werden [18].
