Zusammenfassung
Bei der Behandlung des lokal begrenzten Prostatakarzinoms erfreut sich die radikale Prostatektomie bei den Urologen einer zunehmenden Popularität. Grundlage dafür ist die Überlegung, daß die sicherste Methode zur totalen Ausrottung eines Tumors die vollständige chirurgische Exzision des Neoplasmas zusammen mit dem Organ ist, auf dessen Boden es entstanden ist, und zwar zu einem Zeitpunkt, zu welchem der Tumor noch auf dieses Organ beschränkt ist. Ergebnisse verschiedener Studien haben gezeigt, daß die Behandlungserfolge der radikalen Prostatektomie beim lokal begrenzten Prostatakarzinom die Ergebnisse der Strahlentherapie, und zwar sowohl der externen als auch der interstitiellen Bestrahlung, eindeutig übertreffen (Olsson et al. 1985; Paulson et al. 1982; Whitmore 1982). Aber nicht nur die im Vergleich zur Strahlentherapie höheren Überlebensraten nach radikaler Prostatektomie, sondern auch die zunehmend geringeren Komplikationsraten aufgrund verbesserter Operationstechniken, z.B. der potenzerhaltenden Operationsmodifikation nach Walsh (Catalona 1985; Pontes et al. 1986; Reiner u. Walsh 1979; Walsh u. Donker 1982; Walsh et al. 1982), verstärken den Trend zur operativen Behandlung des Prostatakarzinoms im sog. Frühstadium.
