Abstract
The development of combinations of a diaminopyrimidine and a sulfonamide as a potent antibacterial combination has already been described earlier in this volume. The degree of synergy was pointed out by Bushby and Hitchings (1968) and emphasized by Darrell et al. (1968), who also gave some striking examples of the clinical efficacy of the combination. Synergy between the two components of cotrimoxazole could be demonstrated in vitro over quite a wide range of ratios of one drug to another. Furthermore, the pharmacokinetics were such (see Chap. 10) that although the trimethoprim and sulfonamide might be administered in one particular ratio by mouth, the ultimate levels in the various tissues might be different. In addition, the infecting organisms differed between tissues. Thus, it was at least theoretically possible that whilst a ratio of 1 part trimethoprim to 5 parts sulfamethoxazole might be most appropriate for treating infections of one organ, a ratio of 1:20 might be better elsewhere.
