Indikation, Technik und Komplikationen unterschiedlicher „Docking-Manöver“ beim Segmenttransport der Tibia

Summary

Effective „Docking“ is of fundamental importance to achieve bony union. The transport systems employed were the Ilizarov ring fixator (15 cases) and the AO unilateral fixator (31 cases). The average transport distance was 15.6 days/cm, the average transport distance was 7.7 cm. The following principles are advocated: For „Docking“ with diaphyseal to diaphyseal contact — cancellous bone graft and a dynamic compression plate are placed in the „docking“ region when a unilateral frame is in use. With a ring fixator, there is better stability of the ununited bone fragments and excellent compression provided by the frame. For „docking“ diaphyses to metaphyses, cancellous bone graft is added to the „docking“ region, when the unilateral frame is in use. For ring fixators, the compression provided by the frame is all that is required.

Zusammenfassung

46 Patienten mit Tibiadefekten wurden von 1987–1992 mittels Transportkortikotomie behandelt. 15 Patienten wurden mittels Ringfixateur, 28 Patienten mit dem unilateralen Fixateur der AO behandelt. Die durchschnittliche Transportzeit belief sich auf 15,6 Tage/cm, die durchschnittliche Distanzstrecke betrug 7,7 cm. 1. Diaphyse: in der Diaphyse ist eine hohe Kompression im Kontaktbereich der Fragmente notwendig, die erzielt werden kann durch den Ringfixateur. Bei Einsatz des unilateralen Fixateurs ist meistens eine zusätzliche Osteosynthese durch Kleinfragmentplatte notwendig. 2. Metaphyse: Bei Anwendung des Ringfixateurs und korrektem „Eintauchen” sind keine weiteren operativen Maßnahmen erforderlich, bei Verwendung des unilateralen Systems die zusätzliche Spongiosaplastik.
