Vollhauttransplantate für Hals- und Gesichtsrekonstruktion

Summary

By the use of preoperative expansion, full thickness grafts have been enlarged more than three times the size that allows primary wound closure of the donor area under normal conditions, and the length of rotation flaps from the shoulder thoracic area could be increased 4:1 in relation with the base of the flap, even in random pattern flaps. The cheeks as well as the upper and lower lip have been reconstructed using the frontal part of the neck by cranial advancement with and without expansion; these flaps have also been used for the reconstruction of scar formations in the centre of the frontal neck by means of some kind of a Z-plastic producing a diagonal scar above the ventral neck. The use of rotation or advancement flaps prepared by preoperative expansion is propagated in combined muscle skin defects. In the case of an intact platysma the reconstruction with full thickness preexpanded skin grafts shows the best results.

Zusammenfassung

Die Größe der Vollhauttransplantate kann durch Vordehnung mittels Expander verdreifacht werden. Dies ermöglicht, einen Hebedefekt von über 20 cm Durchmesser primär zu verschließen. Ein Rotationslappen im claviculären Thoraxbereich konnte im Verhältnis 4:1 gegenüber der Lappenbasis vergrößert werden. Die Wangen sowie die Ober- und Unterlippe können unter Verwendung der vorderen Halsregion rekonstruiert werden. Diese Lappen werden auch für die Rekonstruktion von Narbenbildungen in der Mitte des ventralen Halses mittels Z-Plastik, durch die eine diagonale Narbe über dem ventralen Hals entsteht, verwendet. Die präoperative Expansion der umgebenden Haut und die Rekonstruktion mit Vollhaut und Verschiebe-Schwenklappen stellt die für Gesicht und Hals besseren Voraussetzungen dar als die sehr selten indizierten freien Lappen.
