Zusammenfassung
In der medizinischen Versorgung der Patienten mit terminaler Niereninsuffizienz steht immer noch die Hämodialyse zahlenmäßig weit an der Spitze. So wurden 1981 in der Bundesrepublik bei über 11000 Dialysepatienten lediglich 478 Nierentransplantationen durchgeführt, nur etwa 8% der niereninsuffizien-ten Patienten leben mit einem funktionierenden Transplantat. Die EDTA-Stati-stik von 1981 weist innerhalb Europas beträchtliche Unterschiede auf: So wird in Dänemark ungefähr 8mal mehr transplantiert als in der Bundesrepublik (EDTA. 1983). Für diese unterschiedliche Favorisierung und Praxis der Nierentransplantation gibt es sehr viele verschiedene Gründe, so z.B. Unterschiede in der gesetzlichen Regelung der Organentnahme bei Toten, in der gesellschaftlichen und individuellen Einstellung zur Organspende, in der Bedeutung ökonomischer Überlegungen, in der Bevölkerungsdichte, in der Organisationsstruktur der Erfassung von potentiellen Empfängern und gespendeten Organen, in der unterschiedlichen Indikationsstellung und in der Verfügbarkeit ausgebildeter Entnahme- und Transplantationsteams. So gering der zahlenmäßige Stellenwert der Nierentransplantation z.Z. in der Bundesrepublik noch ist, so groß ist andererseits die Bedeutung dieser Therapiemethode in den Hoffnungen eines Großteils der Dialysepatienten. Dies wird wohl jedem, der mit Dialysepatienten zu tun hat, rasch in eindrucksvoller Weise deutlich, und wurde auch in vielen psychologischen Arbeiten, z. B. zum „Dialysestreß“, besonders hervorgehoben (s. auch Kap. VI. 3).
