Increased Digitalis Tolerance in Uremic Patients

Abstract

Only a few years ago, the clinicians were still impressed by a decrease of digitalis tolerance in uremic patients with respect to the daily dose required for induction of over dosage symptoms (Jelliffe 1967, 1968; Christiansen et al., 1973; Jelliffe and Brooker, 1974; Dettli, 1971; Ohnhaus et al., 1974; Kramer, 1974). By the introduction of serial radioimmunologic determinations of the plasma digoxin, we have learned to look at digitalis tolerance from a different aspect, which nowadays may be related to the plasma concentration. During the last years, we had accumulated some evidence (Buckesfeld and Bolte 1974; Kramer, 1977) that digitalis tolerance is in fact increased in uremic patients in relation to the plasma concentration. In order to substantiate these preliminary observations, a prospective study was undertaken to determine the incidence of digitalis toxicity in patients with an elevated plasma digoxin in relation to the plasma creatinine.
