Zusammenfassung
Die flexible Endoskopie des oberen Gastrointestinaltraktes spielt in der Diagnostik des Magen-Darm-Ulkus eine zentrale Rolle. Die endoskopische Inspektion ist, gemessen an den Vorteilen der diagnostischen Aussagekraft der Untersuchung, von vertretbarer Invasivität und bietet den Vorteil, neben einer genauen makroskopischen Beurteilung auch eine histologische Untersuchung von auffälligen Befunden durchführen zu können. Die Untersuchung kann bei einem kooperativen nüchternen Patienten ohne weitere Maßnahmen erfolgen, kann aber auch entweder als Routinekonzept oder, auf Wunsch des Patienten sowie in speziell indizierten Fällen bei ängstlichen Patienten, unter Sedierung durchgeführt werden. Zur Sedierung eignen sich z. B. Benzodiazepane wie Midazolam zwischen 2 und 5 mg i.v. Wegen der respiratorischen Depression sollte letzteres über einen Venenweg injiziert werden, um ggf. das Antidot Anexate geben zu können. Alle Regionen des Ösophagus, Magen und Duodenum sollten inspiziert werden und jede auffällige Läsion, insbesondere des Magens, sollte biopsiert werden, um ein Malignom auszuschließen. Beim gastroduodenalen Ulkus ist eine Biopsieentnahme aus dem Randbereich wichtig, um auch kleinere Karzinome entdecken zu können, die sich durch Biopsie des fibrinösen Ulkusgrundes nicht feststellen lassen würden. Im Rahmen der endoskopischen Inspektion sollte darüber hinaus die Helikobakterbestimmung durch Biopsie zur Routinemaßnahme gehören.
