Is Radical Lymphadenectomy Indicated in Lymph Node-Positive Disease?

Abstract

Several alternative treatments exist for patients with clinically localized prostate cancer who have Gleason grades equal to or less than 7. Though an adequate randomized prospective study comparing these various treatment modalities has not been performed, many series of outcomes of treatment of patients with clinically localized prostate cancer exist. If in fact one of these treatments is more effective than the other, the difference is likely very small and could only be delineated in a very large randomized prospective trial. As is well recognized, these treatments include radical prostatectomy, external beam radiation therapy, and brachytherapy. Fortunately, because of the well-recognized stage migration of prostate cancer associated with the widespread use of prostate-specific antigen (PSA) testing, most patients diagnosed with prostate cancer are in this so-called “good risk” category.
