Peripheral Venous Diseases: The Surgical Approach

Abstract

The goal of the surgical treatment of primary varicose veins is to normalize or at least improve venous hemodynamics. This is accomplished by selectively eliminating all incompetent connections between the deep and superficial venous systems, thereby disrupting the abnormal recirculating flow patterns that have developed. This type of treatment can prevent complications of varicose veins such as varicophlebitis and variceal hemorrhage as well as late sequelae with secondary varicosities and chronic venous insufficiency. Surgical treatment must, however, be provided at a stage when secondary pathology has not yet developed. The surgeon should use tissue-conserving techniques that conform to lymphologic and cosmetic principles. Since the operation cannot cure “connective tissue weakness” based on genetic factors, reticular varices and spider veins may eventually recur but have no hemodynamic significance.
