Struktur und Funktion eines Organs sind untrennbar miteinander verbunden. Als zentrales Stoffwechselorgan des Menschen steht die Leber in ständiger Auseinandersetzung mit exogenen und endogenen Noxen. Zu den wichtigsten Ursachen der Leberschädigung zählen:

Ernährungs- und metabolische-hormonelle Störungen,

Medikamente und Toxine,

Erreger,

Endotoxine,

immunologische Störungen, z. B. primäre Autoimmunerkrankungen oder sekundäre Immunreaktionen bei viralen Infekten, medikamentösen und toxischen Schäden,

genetische Störungen, z. B. Speicherkrankheiten (Bilirubin, Glykogen, Eisen, Kupfer), zystische Fibrose, α1-Antitrypsinmangel,

Kreislaufstörungen, z. B. Ischämie, Reperfusion,

cholestatische Erkrankungen,

metallinduzierte Schäden, z. B. Eisen, Kupfer,

physikalische Schäden, z. B. mechanisches Trauma, ionisierende Strahlen.
