Zusammenfassung
Die zu ca. 80 % von den Hepatozyten und ca. 20 % vom intrahepatischen Gallenwegepithel gebildete Galle (ca. 600–800 ml/Tag) wird über die intra- und extrahepatischen Gallenwege nahrungsabhängig diskontinuierlich ins Duodenum abgegeben, wobei Gallenblase und Ductus cysticus im Nebenschluss liegen. Richtung und Geschwindigkeit des Galleflusses hängen von den Druckdifferenzen im ableitenden Gallenwegsystem einschließlich der Gallenblase ab. Szintigraphische und sonographische Untersuchungen sprechen dafür, dass ständig ein Gallefluss mit wechselnden Richtungen in diesem System stattfindet. Gallenblase mit Ductus cysticus und extrahepatische Gallenwege einschließlich Sphinkter Oddi sind dabei als eine funktionelle Einheit anzusehen. Sie sind von großer Bedeutung für den enterohepatischen Kreislauf der Gallensäuren.
