Zusammenfassung
Die meisten Vergiftungen durch Schlangenbisse in Deutschland sind durch europäische Vipern, insbesondere die Kreuzotter (Vipera berus) verursacht; Bisse durch exotische Schlangen sind selten. Die Giftwirkung unterscheidet sich zwischen den Schlangenspezies. Sie lässt sich unterteilen in eine lokale Gewebstoxizität mit Schmerzen, Schwellung und ggf. Nekrosen, Hämatotoxizität mit pro- und antikoagulatorischen Effekten, Myotoxizität und Neurotoxizität (das zentrale und periphere Nervensystem betreffend). Schwere Komplikationen sind das Kompartmentsyndrom, Blutungen, Rhabdomyolyse, Atemlähmung und Schock. Bisse europäischer Vipern zeichnen sich durch eine ausgeprägte lokale Symptomatik mit Schwellung und Schmerzen aus. Allgemeine Therapieprinzipien sind die Ruhigstellung der betroffenen Extremität und symptomatische Behandlung der Giftwirkung. Eine spezifische Therapie kann mit Antivenin erfolgen, ist aber nicht in jedem Fall notwendig. Essenziell ist die Kontaktaufnahme mit einem Giftinformationszentrum.
