Abstract
In this chapter the authors present a surgical series of clival chordoma approached through the endoscopic transnasal route. Binostril transnasal approach with the endoscope fixed in specially designed holder was performed in all cases. The tumors were removed gross totally in five and subtotally in two without any permanent neurological sequela. The endoscope was especially useful during the removal of lateral and intradural tumor compartment. Literature review is conducted to analyze the removal and complication rate, as well as the short-term follow-up results in comparison with the microscopically operated case series, demonstrating higher gross total removal rate and recurrence-free survival for endoscopic series while the permanent neurological complication was kept low and performance status in good condition. The endoscopic transnasal approach can be considered as a valid option within the treatment armamentarium for the clival chordoma. Rational selection or combination of each modality has to be considered depending on each case.
