Zusammenfassung
Eine prolongierte Störung der Blut-Kammerwasserschranke, wie sie z.B. bei Diabetes mellitus (D.m.) beobachtet werden kann, soll die Ausbildung einer Cataracta secundaria nach extrakapsulärer Kataraktoperation und Kunstlinsenimplantation begünstigen. Wir untersuchten den Einfluß von D.m. auf das Ausmaß des Nachstares. 30 Augen von Diabetikern mit durchschnittlicher Diabetesdauer von 17,3 ± 10,4 Jahren und 30 Augen von stoffwechselgesunden Patienten wurden untersucht. Die Patienten wurden nach Alter (± 5 Jahre), Operateur, Kunstlinsentyp und Nachbeobachtungszeitraum (± 4 Monate) gematcht. Das Ausmaß des Nachstars wurde im Durchschnitt 3,5 Jahre nach Kataraktextraktion und Hinterkammerlinsen-implantation anhand von standardisierten Fotografien ausgeweitet. Die mittlere Nach-starziffer, die nach dem verwendeten Auswertungsschema einen Wert zwischen 0 und 4 annehmen kann, betrug bei den Diabetikern 0,7. In der Kontrollgruppe lag die mittlere Nachstarziffer bei 1,6. Dieser Unterschied war signifikant (Wilcoxon-Vorzeichen-Rang-Test: P < 0,002). Diabetiker zeigten über einen Nachbeobachtungszeitraum von 3,5 Jahren eine signifikant geringere Tendenz zur Ausbildung einer Cataracta secundaria.
Summary
A breakdown of the blood-retinal barrier as can be found in the eyes of individuals with diabetes (d. m.) is believed to enhance and accelerate posterior capsule opacification (PCO). We investigated the influence of d.m. on the severity of PCO. Thirty eyes of diabetic patients with an average of 17.3 ± 10.4 years of diabetes duration were compared to 30 eyes of nondiabetic patients. The patients were closely matched by pairs according to age (± 5 years), surgeon, intraocular lens type (three-piece posterior chamber = PC-IOL), and postoperative period (± 4 months). The PCO score ranging from 0 to 4 was taken at an average of 3.5 years following extracapsular cataract extraction and PC-IOL implantation while utilizing standardized retroillummina-tion photographs. The mean PCO score was 0.7 in diabetics and 1.6 in controls. This difference was statistically significant (Wilcoxon test: P < 0.002). Thus we could not confirm an increased PCO rate in the eyes of diabetics. Other factors than blood-retinal barrier breakdown may play a more important role with the formation of secondary cataract in diabetic eyes.
