Zusammenfassung
In einer klinischen Studie werden Veränderungen der Morphologie des vorderen zentralen Linsenepithels dem Patientenalter sowie dem Reifegrad der Katarakt gegenübergestellt. 108 Patienten werden den Altersklassen I-V zugeordnet und nach dem Reifegrad in Cataracta progrediens und Cataracta (prae)matura unterteilt. Jeweils ein Fragment des vorderen zentralen Linsenepithels (n = 108) wird mit dem Rasterelektronenmikroskop beurteilt. Als Kriterien dienen Unregelmäßigkeiten des Epithelverbandes und Strukturveränderungen der Zellkerne. Bei der Cataracta praesenilis (Altersklassen I und II) sind 13 von 14 Kapselepithel verbänden deutlich geschädigt, bei der Cataracta senilis (Altersklassen III-V) nur 29 von 94. Ein intaktes Epithel liegt im Fall einer senilen Katarakt bei der Cataracta progrediens (n = 76) 62 mal, bei der Cataracta (prae)matura (n = 18) nur 3mal vor. Eine Häufung von Schäden des vorderen zentralen Linsenepithels ist bei frühzeitigem Auftreten der Linsentrübung (P < 0,01) und bei höherem Reifegrad der Katarakt anzutreffen. Der Umfang der Epithel Schädigung könnte für einen stärkeren Einfluß des präsenilen Linsenepithels auf die Kataraktogenese sprechen.
Summary
In a clinical study morphological changes of the anterior central lens epithelium are confronted the age of the patients and the maturity grade of cataract. 108 patients are classified by their age into five classes and by the maturity grade in Cataracta progrediens and Cataracta (prae)matura. In each case a fragment of the anterior central lens epithelium (n = 108) is tested by scanning electron microscopy. Irregularities of the lens epithelial cells and changes in the structure of the nuclei serve as criterions. In Cataracta praesenilis 13 of 14 capsular epitheliums are distinctly defected, in Cataracta senilis only 29 of 94. In the case of senile cataract an intact epithelium occurs in the Cataracta progrediens (n = 76) 62 times, in the Cataracta (praematura (n = 18) only three times. An accumulation of defects of the anterior central lens epithelium is to be seen in the case of occurance of lens opacification in an early age (P < 0,01) and by a higher maturity grade of cataract. The extent of the epithelial damage could suggest the stronger influence of the presenile lens epithelium on the ca-taractogenesis.
