Zusammenfassung
Wird bei Schielpatienten das fixierende Auge verdeckt, so erfolgt eine Einstellbewegung, deren Größe dem Exzentrizitätswinkel des Bildes auf der Netzhaut des schielenden Auges entspricht. Der Mechanismus dieser Einstellbewegung scheint also einer Blickzielbewegung des Normalen ähnlich zu sein.
Nun findet man aber bei manchen Schielpatienten auch dann eine Blicksakkade, wenn das Bild des schielenden Auges mit Hilfe von Prismen bereits auf die Fovea centralis umgelenkt wurde. Diese „Einstellbewegung“ bringt zwar das Auge in Primärstellung, jedoch wird dabei in widersinniger Weise das durch Prismen bereits auf die Fovea projizierte Bild zunächst in die Peripherie verschoben und erst mit einem Gegenruck wieder auf die Fovea zurückgebracht.
Dieses Phänomen scheint uns ein Hinweis darauf, daß die Einstellbewegungen der Schielenden nicht nur retinotop organisiert sind, sondern daß auch ein extraretinales Signal über die Größe des Schielwinkels zur Verfügung steht. Dieses Signal könnte beim raschen Alternieren zwischen der Fixation des rechten und des linken Auges nützlich sein.
Summary
If the fixing eye of a squinting patient is covered, a saccade to take up fixation follows which corresponds to the eccentricity of the image on the retina of the squinting eye. The mechanism of this saccade appears to correspond to a refixation movement in normal subjects.
However, in some strabismus patients, one finds a saccade even though the image on the retina of the squinting eye had already been moved onto the fovea centralis with the aid of prisms. This saccade brings the eye into the primary position and displaces the image of the fixation target — which had already been projected onto the fovea by means of prisms — to a peripheral retinal point, so that the image must be recentered onto the fovea by a second saccade.
We propose to call this to and fro movement “rebound-saccade”. This phenomenon indicates to us that the refixation saccade which occurs with the cover test is not only retinotopically organized. Extraretinal information regarding the angle of squint seems to be available to the ocular motor system. This information may be useful during rapid alternation between fixation of the right and the left eye. The rebound-saccade cannot be explained by anomalous retinal correspondence, as has been suggested by other authors. If the extrafoveal site in the esotropic eye which corresponds with the fovea of the other eye remained the motor zero point for a short while after covering the fixing eye, one would assume that the first saccade in the squinting eye is directed nasally. We find however that the first saccade is consistently directed temporally.
