Nicht-aldosteron-bedingter Mineralocorticoidismus

Zusammenfassung

Vom Conn-Syndrom [1 bis 3] ist der nicht-aldosteronbedingte Mineralocorti-coidismus abzugrenzen. Wie bei jenem ist das klinische Bild gekennzeichnet durch 1. Adynamie, 2. Polydipsie und Polyurie, 3. eine mehr oder weniger ausge-prägte Hypertonic Die Laboruntersuchungen ergeben eine 1. Hypokaliämie und 2. schwere metabole Alkalose. Im Gegensatz zum Conn-Syndrom ist aber 3. die Aldosteronausscheidung vermindert bis normal bei gesteigerter Sekretion der übrigen Mineralocorticoide (DOC, Corticosteron). Den primären nicht-aldosteron-bedingten Mineralocorticoidismus findet man beim DOC- und Corticosteron-bildenden NN-Carcinom [4 bis 9], den sekundären infolge ACTH-Exzeß bei 1. hypophysär-corticotropen Geschwülsten [6, 10 bis 12], 2. ektopischer ACTH-Bildung unreifer corticotroper Geschrwülste [5, 6, 13 bis 23], 3. beim 17α-Hydro-xylasemangel [24 bis 27]. Zusätzliche Leitsymptome können bei sekundärem nichtaidosteronbedingtem Mineralocorticoidismus Pigmentierung, Ödeme, Hirsu-tismus und Steroiddiabetes sein. Im folgenden wird über vier derartige Fälle berichtet.
