Abstract

Surgical considerations after primary systemic therapy will depend on which of the two main reasons for choosing primary chemotherapy prevails. Most European studies seek to decrease the size of some larger tumors so as to make them acceptable candidates for breast-conserving surgery (BCS). In the National Adjuvant Surgical Breast and Bowel Project (NSABP) study, the aim is to improve disease-free survival and overall survival by testing the hypothesis that existing micrometastases would be more sensitive to primary chemotherapy than they would be following surgical removal of the original tumor. Tumor reduction to permit BCS is a worthwhile but secondary goal.
