Zusammenfassung
Das benigne Prostatasyndrom (BPS) ist charakterisiert durch Miktionsbeschwerden bei Prostatavergrößerung und Blasenauslassobstruktion. Neben hormonellen Faktoren (Verschiebung des Androgen-Östrogen-Gleichgewichts im Alter) sind auch diverse Umweltfaktoren ursächlich an der Entwicklung eines BPS beteiligt. Diagnostisch wichtig sind eine genaue Anamnese, unterstützt durch standardisierte Fragebögen, körperliche Untersuchung, Urin- und Bluttests sowie Abdomensonografie, TRUS und Uroflowmetrie. Ein Prostatakarzinom ist auszuschließen. Zur Therapie stehen diverse Medikamente zur Verfügung: Phytopharmaka (meist ohne genauen Wirkungsnachweis), 5α-Reduktase-Inhibitoren, α1-Adrenozeptor-Antagonisten (α-Blocker) und Muskarinrezeptorantagonisten. Operativ gilt die TURP (transurethrale Elektroresektion der Prostata) als Referenzverfahren, daneben haben die transurethrale Inzision der Prostata und die supra-/retropubischeProstataadenomenukleation sowie als Alternative verschiedene Laserverfahren ihren Stellenwert.
