Zusammenfassung
Die fetale Schilddrüse nimmt ab der 10. bis 12. Gestationswoche über den Natrium-Jod-Symporter aktiv Jod auf und produziert Thyroxin (T4). Der Regelkreis Hypothalamus-Hypophyse-Schilddrüse ist ab der 2. Schwangerschaftshälfte intakt. Der Fetus ist auf seine eigene Hormonbildung angewiesen. Mütterliches Jod passiert die plazentare Schranke; T4 und Trijodthyronin (T3) passieren sie nur zu einem sehr geringem Teil, Thyreotropin (TSH) überhaupt nicht. Liegen bei einer fetalen Hypothyreose hohe materno-fetale Gradienten vor, wird mütterliches T4 diaplazentar transportiert. Diese T4-Spiegel reichen aus, um das fetale zentrale Nervensystem vor den Folgen einer Hypothyreose zu schützen. Normalerweise steigt die T4-Konzentration bis zur Geburt kontinuierlich an, während die T3-Werte bei Geburt nur etwa einem Drittel der mütterlichen Werte entsprechen.
