Zusammenfassung

Zystitis und Urethritis stellen die häufigsten urologischen Entzündungen dar. Charakteristische Symptome wie Algurie, Pollakisurie und urethraler Fluor führen zu der Verdachtsdiagnose. Neben gängigen Erregern wie gramnegativen Enterobacteriaceae bei Zystitis oder Erregern von Geschlechtskrankheiten bei der Urethritis muss, insbesondere bei therapierefraktären oder häufig rezidivierenden Verläufen auch an komplizierende Kausalfaktoren oder seltenere Entitäten gedacht werden. Eine asymptomatische Bakteriurie muss nur bei Schwangeren oder vor Operationen saniert werden. Eine unkomplizierte Zystitis wird bei Frauen mit einer Kurzzeittherapie behandelt. Nach der Sanierung von Kausalfaktoren wie Fisteln zum Genitaltrakt oder einer subvesikalen Obstruktion kann sich die Entzündungsneigung verlieren. Bei häufigen Rezidiven einer unkomplizierten Zystitis können diese durch eine antibiotische oder nicht antibiotische (Impfungen, Östrogene, Mannose, Phytotherapeutika) Langzeitprophylaxe in ihrer Häufigkeit verringert werden.
