Zusammenfassung

Die Pharmakotherapie stellt eine wichtige therapeutische Säule in der Behandlung der weiblichen Harninkontinenz dar. So kommen bei einer Detrusorhyperaktivität orale Anticholinergika, ein ß3-Adrenorezeptoragonist oder Botulinumtoxin A zum Einsatz. Bei einer Belastungsinkontinenz kann Duloxetin die Symptome signifikant verbessern, bei Östrogenmangel sollte eine lokale Substitution erfolgen. Bei hypotoner Urethra oder neurogener Blase ist die operative Sphinkterimplantation, die technisch sehr schwierig und mit verschiedenen Komplikationen verbunden sein kann, indiziert. Bei leichter und mittelgradiger Belastungsinkontinenz ohne große Organsenkung, wird heute primär mit konservativen physiotherapeutischen Mitteln versucht eine operative Therapie zu vermeiden. Zur Verbesserung der aktiven Verschlusskraft wird der Beckenboden trainiert. In den modernen Behandlungskonzepten wird versucht, die Innervation und Durchblutung von Beckenboden und Harnröhre zu verbessern. Ziel einer Blasenmobilisation ist, die passive Drucktransmission zu optimieren.
