Implementing the Heart Team Approach for Optimal Strategy in Myocardial Revascularization and Structural Valve Disease

Abstract

Cardiovascular treatments are based on complex decision making process requiring broad knowledge base spanning the fields of cardiology and heart surgery. While early on in 1970th and 1980th cardiologists and surgeons participated in decision making process later on rather competitive attitudes were adopted and independent decisions by either specialty prevailed. Recently, based on evidence suggesting significant differences in outcome with different modalities of treatment (surgery vs. intervention vs. conservative management) in different groups of patients the critical importance of interdisciplinary exchange and collaboration has been highlighted and team decisions preferred as means to improve the quality of cardiovascular treatments. The chapter reviews some of the key aspects of working of specialists together in a heart team scenario.

Abbreviations

Cognitive skills:

In context with revascularization therapies ability of the operator to perceive relevant data, to pass appropriate judgment and to determine the course of action.

Decision making:

The highest level of cognitive skills. Ability of the operator to decide on optimum strategy based on case specific data.

FAME:

Fractional flow reserve versus angiography for guiding PCI in patients with multivessel evaluation trial comparing revascularization strategies guided by fractional flow reserve versus angiography.

Heart team:

Team of cardiovascular specialists providing cardiovascular treatments based on interdisciplinary consensus regarding the optimum choice of treatment. Typically, team members include cardiac surgeon, interventionist and conventional cardiologist.

SYNTAX:

Synergy between percutaneous coronary intervention with TAXUS and cardiac surgery trial comparing interventional and surgical revascularization strategies in patients with complex multi-vessel disease.

Teamwork:

Interactive transfer of knowledge by specialists with complementary expertise.
