Abstract

Diabetic foot problems remain a serious problem, occupying a substantial number of bed days in hospital and accounting for over 80 % non-traumatic amputations. A multidisciplinary team approach is essential for the management of these problems. Ulceration occurs because of the twin complications of neuropathy and ischaemia. Loss of sensation is the permissive factor allowing ulceration. Establishing the aetiology of the ulceration, and whether any deep infection is present, allows management along the correct pathway. The core of treatment is non-operative with operative treatment reserved for deep infection and recurrent ulceration in the presence of fixed deformity. The Charcot foot represents a particular challenge with the extent of swelling and deformity which occurs. Protective footwear and insoles are essential to prevent re-ulceration which can still occur.
