Zusammenfassung

Das Notfallteam einer Intensivstation wird am späten Nachmittag zu einer Reanimation auf eine allgemeinchirurgische Station gerufen. Das Team findet einen Patienten im Herz-Kreislauf-Stillstand vor, an dem drei Pflegekräfte und eine Schwesternschülerin eine Herz-Lungen-Wiederbelebung durchführen. Der Intensivarzt hat den Eindruck, dass die Reanimation unkoordiniert abläuft. Er übernimmt sowohl die Beatmung des Patienten als auch die Führung des Geschehens. Zuerst weist er allen Anwesenden je eine Aufgabe zu. Wenige Minuten nach dem Beginn der erweiterten Reanimationsmaßnahmen trifft der chirurgische Dienstarzt bei dem Patienten ein. Der Intensivarzt weist ihn in die aktuelle Lage ein und bittet um Unterstützung. Beide Ärzte tauschen sich über die Verdachtsdiagnose »Lungenembolie« aus, die aber nach Hinweis einer Pflegekraft auf eine am Morgen durchgeführte Splenektomie verworfen wird. Unter dem Verdacht eines hämorrhagischen Schocks wird die Reanimation um einen forcierten Volumenersatz erweitert. Dazu wird ein Shaldon-Katheter in die rechte V. jugularis interna eingelegt, 2500 ml Volumenersatz zugeführt und fraktionierte Boli von Suprarenin appliziert.
