Allergenmeidung, symptomatische Pharmakotherapie und Hyposensibilisierung (spezifische Immuntherapie [SIT]) sind die grundlegenden Maßnahmen im Management allergologischer Erkrankungen. Die Korrektur der aberranten Immunregulation durch kontrollierte Applikation ansteigender Allergendosen bei der Hyposensibilisierung besitzt dabei die größte Attraktivität. IgE-vermittelte Atemwegserkrankungen durch bestimmte Allergene und die ebenfalls IgE-vermittelte Hymenopterengiftallergie können heute damit sehr wirksam und, bei Beachtung von Indikationen und Kontraindikationen, auch sicher therapiert werden. Grundlage hierfür sind aussagestarke klinische Studien, die Verfügbarkeit von Allergenzubereitungen hoher Qualität sowie ein breiter Konsensus zu den Modalitäten der Anwendung dieser Therapieform. In den letzten Jahren wurde in Ergänzung zur subkutanen Immuntherapie ein neuer Applikationsweg der Allergene, die sublinguale Immuntherapie (SLIT), erschlossen.
