Unter den von der Niere ausgehenden Tumoren stellen der Wilms-Tumor (Nephroblastom) und seine Varianten im Kindesalter die wichtigste Krankheitsgruppe dar. Dabei sind Tumoren mit günstiger, intermediärer und ungünstiger Histologie hinsichtlich der Therapiestrategien zu unterscheiden. Durch die Anwendung einer risikoadaptierten Therapie gemäß den Vorgaben des aktuellen Therapieprotokolls SIOP/GPOH lassen sich bestmögliche Heilungschancen erzielen und das Ausmaß der zu erwartenden Nebenwirkungen begrenzen. Die Anwendung einer präoperativen Chemotherapie verbessert die Möglichkeiten der vollständigen Tumorresektion (Down-Staging) und verringert zudem die Gefahr einer Tumorruptur. Diese Vorgehensweise macht den Einsatz von Strahlentherapie häufig verzichtbar und hilft somit, Spätfolgen — insbesondere Wachstumsstörungen im Bereich der Wirbelsäule (Skoliose/Kyphose) — zu vermeiden. Aus therapeutischer Sicht sind andere gut- und bösartige Tumoren der Nierenregion und des Urogenitaltraktes vom Wilms-Tumor abzugrenzen. Die wichtigsten Differenzialdiagnosen sowie die entsprechenden Behandlungsoptionen werden aufgezeigt.
