Zusammenfassung
Die chirurgischen Pathologien der Milz umfassen Verletzungen, hämatologische Erkrankungen und seltener fokale Veränderungen. Wenn früher bei einer traumatischen Milzruptur eine Splenektomie vorgenommen wurde, wird heute wegen der unerwünschten Folgen des Milzverlustes (»overwhelming postsplenectomy infection«) nach Möglichkeit eine Organerhaltung angestrebt. Milzerhaltende Eingriffe stellen aber wegen der hohen operativ-technischen Anforderungen eine neue Herausforderung für den Chirurgen dar. Bei verschiedenen gutartigen und malignen hämatologischen Erkrankungen nimmt wegen raffinierterer diagnostischer Verfahren und hochwirksamer Chemotherapie die diagnostische Splenektomie an Bedeutung ab. Die Indikationen zur therapeutischen Splenektomie sind bei Verdrängungssymptomatik durch sehr große Milzen und bei den Folgen des Hypersplenismus mit Anämie und/oder hämorrhagischer Diathese gegeben.
