Anal Considerations: Fournier’s Gangrene

Abstract

Fournier’s gangrene (FG) is a rapidly progressive necrotizing soft tissue infection (NSTI) involving the perineum and the external genitalia. FG is relatively rare and has an annual incidence of approximately 1.6 cases per 100,000 men. While local skin flora and the urogenital tract are associated with FG, the most common bacterial source comes from the anorectum. Survival from FG is dependent on early clinical recognition, aggressive resuscitation, and surgical debridement. Fecal diversion via ostomy may be performed, though its use remains controversial and should be tailored accordingly. Anorectal cancer-related FG may necessitate synchronous primary resection during debridement.
