Clinical Decision Making in Colorectal Surgery

Abstract

Crohn’s disease is a chronic inflammatory disorder. The broad variability in phenotypic presentations makes the treatment of this disease a true multidisciplinary approach. Anorectal fistulae are a frequent manifestation of CD, often requiring a staged operative approach with a combination of aggressive medical management. The operative techniques vary from a simple fistulotomy to complex endorectal advancement flaps, with novel therapies becoming ever more popularized. Despite the treatment employed, the primary focus in the management of perianal CD is to prevent ongoing tissue destruction by controlling sepsis, preserving sphincter integrity, and ultimately maintaining the patient’s continence.
