Treatment of Genito-Pelvic Pain/Penetration Disorder

Abstract

The objective of this chapter is to review strategies for management of genito-pelvic pain/penetration disorder (GPPPD). GPPPD refers to pain or fear of pain with sexual genital contact or vaginal penetration. This broad diagnosis demands a comprehensive evaluation and a multiplatform management strategy. GPPPD is commonly associated with specific diagnoses including localized vulvodynia, vulvar dermatoses, hormonally related atrophy, neuropathy, vaginismus, pelvic floor hypertonus and myalgia, and chronic pelvic pain conditions such as endometriosis and bladder pain syndromes. Typically, multiple diagnoses underlie dyspareunia. Treatment is dependent on identification of all components of pain. Psychological, interpersonal, and cultural dynamics must be addressed. Multidisciplinary management generally combines medication, physical therapy, psychological therapy, and when indicated, surgery. This chapter presents evidence-based treatment modalities for each of the conditions commonly associated with GPPPD.
