Abstract
Catastrophes in medicine are not only objective failures, but personal disasters. In general they are caused by multiple mistakes fitting together until the definite failure occurs. Thus more or less many of these catastrophes can be prevented although to err is human. Examples of medical errors were described and conclusions are drawn to learn from them.
