Microsurgical Approach to Arachnoid Cysts

Abstract

Intracranial arachnoid cysts (ACs) are seen in up to 2.6% of the population. Many of these cysts are asymptomatic; however, a variety of signs and symptoms may be seen, with headache being the most prevalent. Symptomatic ACs warrant surgical treatment for improved quality of life, and, for patients presenting with hydrocephalus, intervention may be a life-saving measure. Surgical treatment options for ACs include craniotomy with cyst wall marsupialization or fenestration, neuroendoscopic cyst fenestration, or cystoperitoneal shunt insertion. Across many studies, good clinical and radiographic outcomes are seen using a microsurgical technique. Reoperation or secondary treatment may be necessary in a subset of these patients. Microsurgical AC fenestration or excision is an effective surgical treatment option in the appropriately selected patient.
