Thoracic Applications Per Oral Plication of the Esophagus (POPE)

Abstract

Per oral plication of esophagus (POPE) is a novel endoscopic intervention to address redundancy in megaesophagus or dilated neoesophageal conduit. It is indicated when contrast imaging shows ‘sump’ formation, areas where tortuosity or saccular dilation allows contrast to pool, rather than flow through the organ with gravity. Other causes for delayed emptying such as pseudoachalaisa or incompetent pylorus must also be ruled out. Traditionally this problem requires esophagectomy or surgical revision of the neoesophageal conduit. The following chapter details the pathogenesis of indications for POPE, describes how the procedure is performed and reviews specific technical considerations.
