Endoscopic Submucosal Dissection for Early Malignant Epithelial Neoplasms of the Digestive Tract in Frail and Elderly Patients

Abstract

Endoscopic submucosal resection (ESD) is an effective alternative to surgery in managing early digestive tract tumors and, in Japan, represents the gold standard of treatment. ESD can be considered a relatively safe technique when performed in real-life practice in a tertiary endoscopic center. In this chapter, we will describe indications, technical details, and complications of the procedure. Complications can occur in less than 10% of cases, and the most frequent are perforation and bleeding. In rare cases, major complications, such as esophageal perforation, can be managed endoscopically but may require emergency surgery. In the case of planning endoscopic resection, accurate histology should be evaluated, as well as the size and depth of the tumor, particularly in cases of esophageal and gastric tumors.
